Acute bronchitis, also known as a chest cold, is short-term inflammation of the bronchi (large and medium-sized airways) of the lungs.[1][2] The most common symptom is a cough.[2] Other symptoms include coughing up mucuswheezingshortness of breathfever, and chest discomfort. The infection may last from a few to ten days.[1] The cough may persist for several weeks afterwards with the total duration of symptoms usually around three weeks.[1][2] Some have symptoms for up to six weeks.[3]

In more than 90% of cases the cause is a viral infection.[2] These viruses may be spread through the air when people cough or by direct contact. Risk factors include exposure to tobacco smoke, dust, and other air pollution.[1] A small number of cases are due to high levels of air pollution or bacteria such asMycoplasma pneumoniae or Bordetella pertussis.[2][4] Diagnosis is typically based on a person's signs and symptom.[5] The color of the sputum does not indicate if the infection is viral or bacterial. Determining the underlying organism is typically not needed.[2] Other causes of similar symptoms include asthma,pneumoniabronchiolitisbronchiectasis, and COPD.[2][6] A chest X-ray may be useful to detect pneumonia.[2]Bronchitis is an inflammation of the bronchial tubes, the airways that carry air to your lungs. It causes a cough that often brings up mucus. It can also cause shortness of breath, wheezing, a low fever, and chest tightness. There are two main types of bronchitis: acute and chronic.

Most cases of acute bronchitis get better within several days. But your cough can last for several weeks after the infection is gone.

The same viruses that cause colds and the flu often cause acute bronchitis. These viruses spread through the air when people cough, or though physical contact (for example, on unwashed hands). Being exposed to tobacco smoke, air pollution, dusts, vapors, and fumes can also cause acute bronchitis. Less often, bacteria can also cause acute bronchitis.

To diagnose acute bronchitis, your health care provider will ask about your symptoms and listen to your breathing. You may also have other tests.

Treatments include rest, fluids, and aspirin (for adults) or acetaminophen to treat fever. A humidifier or steam can also help. You may need inhaled medicine to open your airways if you are wheezing. Antibiotics won't help if the cause is viral. You may get antibiotics if the cause is bacterial.

What Is Bronchitis?

People who have bronchitis often have a cough that brings up mucus. Mucus is a slimy substance made by the lining of the bronchial tubes. Bronchitis also may cause wheezing (a whistling or squeaky sound when you breathe), chest pain or discomfort, a low fever, and shortness of breath.

Overview

The two main types of bronchitis are acute (short term) and chronic (ongoing).

Acute Bronchitis

Infections or lung irritants cause acute bronchitis. The same viruses that cause colds and the flu are the most common cause of acute bronchitis. These viruses are spread through the air when people cough. They also are spread through physical contact (for example, on hands that have not been washed).

Sometimes bacteria cause acute bronchitis.

Acute bronchitis lasts from a few days to 10 days. However, coughing may last for several weeks after the infection is gone.

Several factors increase your risk for acute bronchitis. Examples include exposure to tobacco smoke (including secondhand smoke), dust, fumes, vapors, and air pollution. Avoiding these lung irritants as much as possible can help lower your risk for acute bronchitis.

Most cases of acute bronchitis go away within a few days. If you think you have acute bronchitis, see your doctor. He or she will want to rule out other, more serious health conditions that require medical care.

Chronic Bronchitis

Chronic bronchitis is an ongoing, serious condition. It occurs if the lining of the bronchial tubes is constantly irritated and inflamed, causing a long-term cough with mucus. Smoking is the main cause of chronic bronchitis.

Viruses or bacteria can easily infect the irritated bronchial tubes. If this happens, the condition worsens and lasts longer. As a result, people who have chronic bronchitis have periods when symptoms get much worse than usual.

Chronic bronchitis is a serious, long-term medical condition. Early diagnosis and treatment, combined with quitting smoking and avoiding secondhand smoke, can improve quality of life. The chance of complete recovery is low for people who have severe chronic bronchitis.


Prevention is by not smoking and avoiding other lung irritants. Frequent hand washing may also be protective.[7] Treatment of acute bronchitis typically involves rest, paracetamol (acetaminophen), and NSAIDs to help with the fever.[3][8] Cough medicine has little support for its use and is not recommended in children less than six years of age.[2][9] There is tentative evidence that salbutamol may be useful in those with wheezing; however, it may result in nervousness.[2][10] Antibiotics should generally not be used.[11] An exception is when acute bronchitis is due to pertussis. Tentative evidence supports honeyand pelargonium to help with symptoms.[2]

Acute bronchitis is one of the most common diseases.[3][12] About 5% of adults are affected and about 6% of children have at least one episode a year.[6][13] It occurs more often in the winter.[6] More than 10 million people in the United States visit a doctor each year for this condition with approximately 70% receiving antibiotics, most of which are not needed.[3] There are efforts to decrease the use of antibiotics in acute bronchitis.[12]

Signs and symptoms

Bronchitis may be indicated by an expectorating cough, shortness of breath (dyspnea), and wheezing. On occasion, chest painsfever, and fatigue or malaise may also occur. In addition, bronchitis caused by Adenoviridae may cause systemic and gastrointestinal symptoms as well. However, the coughs due to bronchitis can continue for up to three weeks or more even after all other symptoms have subsided.[citation needed]

What Are the Signs and Symptoms of Bronchitis?

Acute Bronchitis

Acute bronchitis caused by an infection usually develops after you already have a cold or the flu. Symptoms of a cold or the flu include sore throat, fatigue (tiredness), fever, body aches, stuffy or runny nose, vomiting, and diarrhea.

The main symptom of acute bronchitis is a persistent cough, which may last 10 to 20 days. The cough may produce clear mucus (a slimy substance). If the mucus is yellow or green, you may have a bacterial infection as well. Even after the infection clears up, you may still have a dry cough for days or weeks.

Other symptoms of acute bronchitis include wheezing (a whistling or squeaky sound when you breathe), low fever, and chest tightness or pain.

If your acute bronchitis is severe, you also may have shortness of breath, especially with physical activity.

Chronic Bronchitis

The signs and symptoms of chronic bronchitis include coughing, wheezing, and chest discomfort. The coughing may produce large amounts of mucus. This type of cough often is called a smoker's cough.

Cause

Acute bronchitis can be caused by contagious pathogens, most commonly viruses. Typical viruses include respiratory syncytial virusrhinovirusinfluenza, and others. Bacteria are uncommon pathogens but may include Mycoplasma pneumoniaeChlamydophila pneumoniaeBordetella pertussisStreptococcus pneumoniae, and Haemophilus influenzae.[citation needed]

  • Damage caused by irritation of the airways leads to inflammation and leads to neutrophils infiltrating the lung tissue.
  • Mucosal hypersecretion is promoted by a substance released by neutrophils.
  • Further obstruction to the airways is caused by more goblet cells in the small airways. This is typical of chronic bronchitis.
  • Although infection is not the reason or cause of chronic bronchitis, it is seen to aid in sustaining the bronchitis.

Diagnosis

physical examination will often reveal decreased intensity of breath sounds, wheezing, rhonchi, and prolonged expiration. Most physicians rely on the presence of a persistent dry or wet cough as evidence of bronchitis.[citation needed]

What is bronchitis?

Bronchitis means that the tubes that carry air to the lungs camera.gif (the bronchial tubes) are inflamed and irritated. When this happens, the bronchial tubes swell and produce mucus. This makes you cough.

There are two types of bronchitis:

Most healthy people who get acute bronchitis get better without any problems. But it can be more serious in older adults and children and in people with other health problems, especially lung diseases such asasthma or COPD. Complications can include pneumonia and repeated episodes of severe bronchitis.

What causes acute bronchitis?

Acute bronchitis is usually caused by a virus. Often a person gets acute bronchitis a few days after having an upper respiratory tract infectionsuch as a cold or the flu. Sometimes acute bronchitis is caused bybacteria.

Acute bronchitis also can be caused by breathing in things that irritate the bronchial tubes, such as smoke. It also can happen if a person inhales food or vomit into the lungs.

What are the symptoms?

The most common symptom of acute bronchitis is a cough that usually is dry and hacking at first. After a few days, the cough may bring up mucus. You may have a low fever and feel tired.

Most people get better in 2 to 3 weeks. But some people continue to have a cough for more than 4 weeks.

If your symptoms get worse, such as a high fever, shaking chills, chest orshoulder pain, or shortness of breath, you could have pneumonia. Pneumonia can be serious, so it's important to see a doctor if you feel like you're getting sicker. How is acute bronchitis diagnosed?

Your doctor will ask you about your symptoms and examine you. This usually gives the doctor enough information to find out if you have acute bronchitis.

In some cases, you may need a chest X-ray or other tests to make sure that you don't have pneumonia, whooping cough, or another lung problem. This is especially true if you've had bronchitis for a few weeks and aren't getting better. More testing also may be needed for babies, older adults, and people who have lung disease (such as asthma orCOPD) or other health problems.

How Is Bronchitis Diagnosed?

Your doctor usually will diagnose bronchitis based on your signs and symptoms. He or she may ask questions about yourcough, such as how long you've had it, what you're coughing up, and how much you cough.

Your doctor also will likely ask:

  • About your medical history
  • Whether you've recently had a cold or the flu
  • Whether you smoke or spend time around others who smoke
  • Whether you've been exposed to dust, fumes, vapors, or air pollution

Your doctor will use a stethoscope to listen for wheezing (a whistling or squeaky sound when you breathe) or other abnormal sounds in your lungs. He or she also may:

  • Look at your mucus to see whether you have a bacterial infection
  • Test the oxygen levels in your blood using a sensor attached to your fingertip or toe
  • Recommend a chest x raylung function tests, or blood tests

    How Is Bronchitis Treated?

    The main goals of treating acute and chronic bronchitis are to relieve symptoms and make breathing easier.

    If you have acute bronchitis, your doctor may recommend rest, plenty of fluids, and aspirin (for adults) or acetaminophen to treat fever.

    Antibiotics usually aren't prescribed for acute bronchitis. This is because they don't work against viruses—the most common cause of acute bronchitis. However, if your doctor thinks you have a bacterial infection, he or she may prescribe antibiotics.

    A humidifier or steam can help loosen mucus and relieve wheezing and limited air flow. If your bronchitis causes wheezing, you may need an inhaled medicine to open your airways. You take this medicine using an inhaler. This device allows the medicine to go straight to your lungs.

    Your doctor also may prescribe medicines to relieve or reduce your cough and treat your inflamed airways (especially if your cough persists).

    If you have chronic bronchitis and also have been diagnosed with COPD (chronic obstructive pulmonary disease), you may need medicines to open your airways and help clear away mucus. These medicines include bronchodilators (inhaled) and steroids (inhaled or pill form).

    If you have chronic bronchitis, your doctor may prescribe oxygen therapy. This treatment can help you breathe easier, and it provides your body with needed oxygen.

    One of the best ways to treat acute and chronic bronchitis is to remove the source of irritation and damage to your lungs. If you smoke, it's very important to quit.

    Talk with your doctor about programs and products that can help you quit smoking. Try to avoid secondhand smoke and other lung irritants, such as dust, fumes, vapors, and air pollution.

    Lifestyle Changes

    The most important step is to not start smoking or to quit smoking. Talk with your doctor about programs and products that can help you quit.

It also includes lean meats, poultry, fish, and fat-free or low-fat milk or milk products. A healthy diet also is low in saturated fat, trans fat, cholesterol, sodium (salt), and added sugar.

    Ongoing Care

    See your doctor regularly and take all of your medicines as prescribed. Also, talk with your doctor about getting a yearly flu shot and a pneumonia vaccine.

    If you have chronic bronchitis, you may benefit from pulmonary rehabilitation (PR). PR is a broad program that helps improve the well-being of people who have chronic (ongoing) breathing problems.

    People who have chronic bronchitis often breathe fast. Talk with your doctor about a breathing method called pursed-lip breathing. This method decreases how often you take breaths, and it helps keep your airways open longer. This allows more air to flow in and out of your lungs so you can be more physically active.

    To do pursed-lip breathing, you breathe in through your nostrils. Then you slowly breathe out through slightly pursed lips, as if you're blowing out a candle. You exhale two to three times longer than you inhale. Some people find it helpful to count to two while inhaling and to four or six while exhaling.Image result for acute bronchiolitis

 

 

Prevention

An oral whole cell nontypeable Haemophilus influenzae vaccine may protect against the disease.[14]

Treatment

Most cases are self-limited and resolve themselves in a few weeks.

Antibiotics

Evidence does not support the general use of antibiotics in acute bronchitis.[11] Acute bronchitis should not be treated with antibiotics unless microscopic examination of the sputum reveals large numbers of bacteria. While antibiotics speed up resolution of the cough by about 12 hours there is a greater risk of gastrointestinal problems and no change in longer term outcomes.[15] Antibiotics use also leads to the promotion of antibiotic-resistant bacteria, which increase morbidity and mortality.[16]

Smoking cessation

To help the bronchial tree heal faster and not make bronchitis worse, smokers should quit smoking completely.[citation needed]
